PASSHE 2011 W-2 INFORMATION

STATE EMPLOYEE ASSISTANCE PROGRAM (SEAP)

SEAP is free and available to you and your family 24/7 to help you deal with emotional or family concerns, alcohol or drug abuse, depression and other behavioral health matters, or financial and legal issues. Call 1-800-692-7459 (TDD 1-800-842-4306).

If you have questions about SEAP, please email RA-SEAPOptions@state.pa.us. Read previous newsletters and find more information about SEAP within ESS (www.myWorkplace.state.pa.us). You can also learn more about many behavioral health topics by visiting the
United Behavioral Health website and using “Pennsylvania” as your access code.

ARTICLES FROM SEAP  NEWLY UPDATED!
“Recognizing the Warning Signs of Autism” (also in Spanish) provides information about autism spectrum disorders and how to recognize their early signs and symptoms. The article also provides links to additional resources to learn more about autism.

RESIDENCY CERTIFICATION FORM
You may have received a “Residency Certification Form” in the mail at home recently as a result of the implementation of Pennsylvania’s new Act 32. While the act changes the way local earned income tax is collected, it should be largely invisible to our faculty and staff. We do not need the form to be returned to us, as we already have the required information on file for all current employees. As long as your address as shown in Employee Self Service is correct, there’s nothing else you need to do. The new form has been posted on the HR website and is available in the office, for any employee with an address change.
